Reason for waiver

I am requesting an exemption from the Cleary University Housing Policy because of the following reason (s):

I will live at my home address above and live with my parents and will commute daily and the commute is less than 25 miles one wayI will have earned 60 cumulative credits by the beginning of the upcoming academic yearI am or will be 23 years old prior to the first day of classesI am married and live with my spouseI am a single parent and living with my dependent child(ren)Extraordinary circumstances: (Supporting documentation attached)

You may need to provide proof of your reason for exemption. (i.e. marriage certificate, proof of parent/guardian residence).

Proof of Dependents - Accepted documents include: (copy of birth certificate or copy of tax form)

Proof of Residence with Parent -

Due to the current regulations with COVID-19 Cleary University will not require the Notarized Letter of Residency.

However, in order to complete the process of submitting the waiver, the student will need to submit a digital copy of their driver’s license, state ID, or passport with their permanent residency by attaching it below. A bill, letter, etc. with the name and mailing address will not be accepted as a permanent address.
